Who issues a salary certificate
Your employer does. A bank, a landlord or a consulate will accept it only on the company's letterhead, signed by an authorised signatory and stamped. That is why this page gives you the wording rather than a document: a generated certificate would be refused at best, and treated as a forgery at worst.
What it usually has to say
Your name and nationality, your position, the date you joined, your salary broken into basic and allowances, and the purpose it was requested for. Banks in particular look for the basic figure, because that is what several of their calculations run on.
